Insurance/Referrals
Agapé Physical Therapy accepts the following insurances for physical therapy coverage:
- Preferred Care
- Blue Choice
- Blue Cross Blue Shield
- Medicare Part B
- Aetna
- United Health Care
- Tricare
- Most Workers' Compensation
- Motor Vehicle Accident (MVA)
- Most Private Insurances
Many of the above listed insurances require a physical therapy referral. You can contact your physician's office to determine if you need a referral. Please be sure to have the referral ready on the day of your initial evaluation.
Direct Access in New York State: New York State no longer requires that a patient have a prescription for physical therapy services, except for MVA or Worker's Compensation cases. If you have any questions about direct access, please contact one of our locations.
