As Executive Director at Agape, my role is to ensure 5 star customer service at all of our Agape locations. My name is Hannah Wolfanger PT, DPT and I am Daemen College graduate. I enjoy traveling to new places and gaining unique experiences. I began my career as a travel physical therapist through which was able to experience various clinic environments and physical therapy treatment styles and techniques. I decided to return to New York in early 2020, and I was lucky enough to find a new home at the Agape clinic in Brockport. I have a passion for working with athletes in sport specific exercises to not only reduce risk of injury, but to improve their ability and skill in their chosen sport.

I completed my last clinical rotation of PT school at our Brockport location which led me to work for Agape. I have always wanted to be a physical therapist working in the community where I live and this job allows me to do just that. I love to incorporate core stabilization into my treatment plans as much as possible as the core is the foundation of all movement. I am a huge sports fan and have played soccer, basketball, and lacrosse through highs school, as well as playing soccer in college. I enjoy looking at movement dysfunction in patients and correcting underlying movement impairments to return patients to pain free activity. I love being a physical therapist because it offers the chance to spend time with patients and and progress them on the road to recovery.

Outside of work, I am an avid sports fan, movie/show binge watcher, and DIY home remodeler. The reason I became a physical therapist was to be able to work together with individuals to help them get back to the things they love to do without restriction.

Hello all! My name is JP Horan, originally from the Albany area, but was brought to Rochester to study physical therapy. In May of 2017, I received my Doctorate of Physical Therapy from Nazareth College of Rochester. Along with working full-time at Agape PT, I am a per diem therapist at Highland Hospital, and a wellness coach at the Southeast YMCA in Pittsford. Aside from work, I am an avid sports fan (basketball being my favorite), a big time movie watcher, and love to enjoy the outdoors- hiking, swimming, kayaking, you name it!

The reason I wanted to become a physical therapist is pretty simple. I want to be the guide for others on their path to recovery. Whether you sprained your ankle, battle chronic pain, live with neurological diagnoses, or are just out of surgery, I want to be the one to help you reach your goals and full potential. The word, “Agape”, is a Greek word meaning, “the highest form of care”, and I look to represent that to the best of my abilities. I’m looking forward to meeting you!

I graduated with my BS in natural sciences as well as my Doctorate of Physical therapy from Daemen College in Amherst New York in 2019. I was born and raised in Rochester, NY and attended Gates-Chili HS where I played football, baseball, and I competitively swam. I decided to become a physical therapist after tearing my ACL playing football in high school, and going through my own bout of physical therapy. My name is Justin Farnsworth and I joined Agape in February of 2018. I received my Doctor of Physical Therapy (DPT) from SUNY Upstate Medical University in Syracuse, NY. Before this, I was an All-American soccer player while getting my degree in Biology at Houghton College.

I am a Board Certified Sports Clinical Specialist (SCS) which is a distinction held by less than 5% of Physical Therapists nationally. I am also a Certified Strength and Conditioning Specialist (CSCS) and an Active Release Technique (ART) LE certified practitioner. Before moving back to Rochester, I practiced in Arizona and New York City and have had the opportunity to spend time with some of the country’s top healthcare professionals and surgeons. Throughout my career I have been able to work with high-level athletes at the Olympic, professional, collegiate and youth levels. While in NYC, I lectured regularly on anterior cruciate ligament reconstruction (ACL) rehabilitation and injury prevention in overhead throwers.

I am passionate about helping every individual not only achieve but exceed their health and wellness goals.

I have a particular passion for sports performance training and physical therapy, in particular with golfers of all abilities. I love PT because I love movement, and seeing how it can help achieve the highest quality of life. I grew up in Kansas City, Missouri and earned my doctor of physical therapy degree from Wichita State University in Wichita, Kansas. I worked in Scottsdale, Arizona for four years before relocating to Rochester in 2016. I have worked for Agape since Spring 2018 and enjoy connecting with my patients, finding out what motivates them, and helping them get back to the activities they love. I enjoy treating all conditions and am especially passionate about helping people recover after knee and shoulder surgeries. In my free time I love being outdoors with my wife and son.

Nate joins Agape after spending the past three years working for the UHS Hospital System in Binghamton, NY. He became a board certified orthopedic clinical specialist in June of 2020. Prior to UHS, he attended Stony Brook University for Physical Therapy school where he spent time doing clinical rotations in Boston, MA and Greenville, SC. For undergraduate, he attended Le Moyne College and obtained a Bachelors in Biology while playing Division I & II baseball. Treatment Philosophy: "My treatment approach is to not only help my patients become pain-free but help them become as strong and resilient as possible. To quote Mark Rippitoe, 'Stronger people are harder to kill, and more useful in general'."

After working in various parts of the medical field for the last 20ys, I could not be more happy to now be a part of the AGAPE family. It is a wonderful feeling for me to be able to help someone else in their time of need. To take that extra step could mean a mile for someone else.

When i am not working, my 9yr old son keeps me busy with making new lego creations, any pretty much anything outdoors. In the summer we enjoy going to our cottage in Cape Vincent, NY.
