My name is Gregory Van Gorden, PT, DPT. I grew up in Vestal, NY and went to Nazareth College undergrad and graduate work. After graduating in 2015 with my Doctorate of Phyiscal Therapy I began working for Agape. I have had the pleasure of working in both our Gates and Chili locations but have recently been promoted to the Assistant Clinic Coordinator Brockport clinic. I am excited to begin serving the Brockport community!

I graduated from D'Youville College with my clinical Doctorate in Physical Therapy, and the University at Buffalo with my Bachelor degree in Exercise Science. I have a passion for orthopedic rehabilitation as well as sports performance training. Hello! My name is Sarah Pericozzi, PT, DPT and and I started working for Agape in May of 2017. Originally I am from Buffalo, NY and graduated from D'Youville College with my Bachelor's in Exercise Science and then graduated from Nazareth College with my Doctorate of Physical Therapy in 2017. My favorite part about my job is teaching my patients about an aspect of their health they didn't know about and to see someone's outlook on life change when they start to feel better and are not defined by their pain. I love hiking, running, and spending time with my dog. I am big into yoga and am often seen using it with my patients in the clinic. I also plan to start teaching classes to the community over in our fully stocked Chili studio space!

Hi, my name is Renee Yerger and I am a Licensed Massage Therapist. I began working at Agape in January as a receptionist in each of our clinics and have since helped build the massage therapy program as part of the new Agape Health and Wellness team. I am a graduate of SUNY Fredonia, where I received my Bachelor’s in Exercise Science and attended Onondaga School of Therapeutic Massage in Rochester for my massage therapy education. I am passionate about helping others achieve relaxation and relief from sore muscles and fatigue. I offer Swedish, Deep Tissue, and Chair Massage. I look forward to helping you relax and feel rejuvenated!

My name is Adam, I am originally from the Syracuse area and graduated with a Doctorate in Physical Therapy from Nazareth College in 2014. I completed my last clinical rotation of PT school at our Brockport location which led me to work for Agape. I have always wanted to be a physical therapist working in the community where I live and this job allows me to do just that. I love to incorporate core stabilization into my treatment plans as much as possible as the core is the foundation of all movement. I completed my last clinical rotation of PT school at the Chili location of Agape which led me to pursue beginning my career here. I played golf for four years at Nazareth College and am an avid sports fan. Since my junior year in high school I knew I wanted to be a physical therapist and use my skills to help other lives the lives they want. Hello all! My name is JP Horan, originally from the Albany area, but was brought to Rochester to study physical therapy. In May of 2017, I received my Doctorate of Physical Therapy from Nazareth College of Rochester. Along with working full-time at Agape PT, I am a per diem therapist at Highland Hospital, and a wellness coach at the Southeast YMCA in Pittsford. Aside from work, I am an avid sports fan (basketball being my favorite), a big time movie watcher, and love to enjoy the outdoors- hiking, swimming, kayaking, you name it!

The reason I wanted to become a physical therapist is pretty simple. I want to be the guide for others on their path to recovery. Whether you sprained your ankle, battle chronic pain, live with neurological diagnoses, or are just out of surgery, I want to be the one to help you reach your goals and full potential. The word, “Agape”, is a Greek word meaning, “the highest form of care”, and I look to represent that to the best of my abilities. I’m looking forward to meeting you!

I'm originally from a small town called Holland Patent, NY and moved out to Rochester to attend Nazareth College. While in college I was a member of the Nazareth College Swimming and Diving team. In my free time, I enjoy swimming, running, being outdoors and spending time with family and friends. Coming from my athletic and physical therapist background I am always promoting an active healthy lifestyle. My name is Mike Hughes and I am a graduate of the Doctor of Physical Therapy program at the University at Buffalo. I have always been very passionate about physical therapy and providing my patients with the most effective and compassionate care I can. I am an avid sports fan and enjoy working with all types of athletes and getting them back to their peak performance level.

My name is Joe Krokenberger and I graduated from the University at Buffalo Doctor of Physical Therapy program in 2016. I have always been interested in physical therapy, athletic performance and human movement. Growing up I was a competitive freestyle skier and frequently would help teammates break down complex movements to improve their skills. Outside of work I also enjoy working on my car and fixing/building things. I have a very mechanical mindset and way of thinking that helps my patients decrease pain and increase quality of life through optimized movement.

My name is Brett Stokoe and I have been working at Agape since September of 2016.I am originally from Cortland, NY and I graduated with my Doctorate in Physical Therapy from Nazareth college in 2015. I am a huge sports fan and have played soccer, basketball, and lacrosse through highs school, as well as playing soccer in college. I enjoy looking at movement dysfunction in patients and correcting underlying movement impairments to return patients to pain free activity. I love being a physical therapist because it offers the chance to spend time with patients and and progress them on the road to recovery.

I was born and raised in Rochester, NY and received my Doctor of Physical Therapy in 2016 from Ithaca College. I have experience as a Travel Physical Therapist having spent the first year of my career working in both outpatient rehabilitation centers and inpatient hospital acute care settings around the country. Although I loved experiencing new places, meeting new people, and the overall lifestyle of travel, I decided to stop traveling and come home to be closer to family and friends. My highest priority as a clinician is not only to provide quality patient specific care but to empower individuals to believe in themselves and achieve their optimal potential. While rehabilitation equipment can be helpful, I believe that my most important tools are my eyes/ears, hands, and inter-professional communication/collaboration. I value and utilize hands-on manual therapy techniques including spinal/peripheral joint mobilization/manipulation, soft tissue mobilization/myofascial release, and myofascial decompression in conjunction with corrective exercise to provide my patients with a comprehensive plan of care. I enjoy working out, hiking, camping, and any time spent time with family and friends.

My name is Justin Farnsworth and I joined Agape in February of 2018. I received my Doctor of Physical Therapy (DPT) from SUNY Upstate Medical University in Syracuse, NY. Before this, I was an All-American soccer player while getting my degree in Biology at Houghton College.

I am a Board Certified Sports Clinical Specialist (SCS) which is a distinction held by less than 5% of Physical Therapists nationally. I am also a Certified Strength and Conditioning Specialist (CSCS) and an Active Release Technique (ART) LE certified practitioner. Before moving back to Rochester, I practiced in Arizona and New York City and have had the opportunity to spend time with some of the country’s top healthcare professionals and surgeons. Throughout my career I have been able to work with high-level athletes at the Olympic, professional, collegiate and youth levels. While in NYC, I lectured regularly on anterior cruciate ligament reconstruction (ACL) rehabilitation and injury prevention in overhead throwers.

I grew up in Chili but moved to Ithaca in 2011 to attend Ithaca College where I received a Bachelor’s of Science in Biology with minors in Honors and Psychology. After graduating I returned to Rochester where I will remain as I take a few years off of my education, however, I plan to apply to medical school within the next two years. My name is Natalia Farnsworth. Prior to joining the Agape team, I practiced physical therapy in Arizona and in NYC; where I worked with a variety of patients particularly the athletic population and the women's health population. In addition to my background working with athletes, I specialize in treating pelvic floor issues ranging from pregnancy, post-partum and pelvic pain. As Executive Director, my role is to foster our 5 star customer service and exceptional care at each of our locations throughout Rochester.

Hi my name is Christopher Greene and I began practicing at Agape in December 2016. I received my Doctorate in PT from Nazareth College of Rochester NY. I’ve practiced physical therapy internationally in India, Asia in my studies to hone in on my most useful skills with new obstacles. I love PT because I love movement, and seeing how it can help achieve the highest quality of life. Without PT I wouldn’t be able to still play my favorite sports including basketball and hockey, and I’d like to help others achieve the same goal!
